In exchange for being allowed to participate in the dog training activities offered by FWOTC, my signature below and/or my voluntary participation in the activities indicates my understanding of those risks, and I do hereby release Fort Wayne Obedience Training Club and its members, directors, trustees, officers, employees, independent contractors, agents and assigns from any and all liability, damages, costs and expenses arising out of or relating to bodily or psychological injury, loss of life or personal property or bodily injury or loss of life to either me or my animal companion that may occur as a result of my voluntary participation in any and all activities and classes associated with FWOTC. Further, I shall indemnify, defend, and hold harmless FWOTC from any and all suits, actions and claims by any third party, including but not limited to, claims for personal injury, including death, or property arising out of or related to my voluntary participation in any and all activities and classes associated with FWOTC, including but not limited to, damage occurring on or about the dog training building and while on the premises or any other property which is used for the purposes of training or exhibiting dogs. Further, FWOTC is not responsible for any property of the member or guests which may be lost, damaged, stolen or for any loss thereof occasioned by fire, the elements or other casualty. All property of the member shall be within the premises of FWOTC at the members’ and guests’ sole risk.
